In the Spring Semester of the 2024-2025 Academic Year, students who will receive practical training in healthcare institutions as part of the applied course must carefully read, fill out, and sign the "Applied Course Acceptance Form" after obtaining it in three copies, double-sided on a single page. Then, they must have the other parts of the form signed by the authorities and submit one copy of the form along with a photocopy of the occupational health and safety certificate that is valid and compatible with the hazard class to their advisor by the end of working hours on Friday, January 10, 2025, in exchange for a signature. Applications of students who do not submit the documents by the specified date will not be considered.
NOTE: Students who will conduct practical training in healthcare institutions affiliated with the Burdur Provincial Health Directorate must have the forms they filled out and signed (there is no need to apply to have them signed by the Burdur Provincial Health Directorate) signed only by the Head of the Department and submit all three copies to their advisor by the end of working hours on Friday, January 10, 2025, in exchange for a signature. Applications of students who do not submit the documents by the specified date will not be considered.
