In the 2023-2024 Academic Year Spring Semester, students who will receive practical training in health institutions within the scope of applied courses must carefully read, fill out, and sign the "Applied Course Acceptance Form" after receiving it as four copies on a single page, front and back. Then, they must have the other parts of the form signed by the authorities and submit two copies to their advisor by the end of working hours on Friday, January 27, 2024 in exchange for a signature. Applications of students who do not submit their documents by the specified date will not be considered.
NOTE: Students who will receive practical training in health institutions affiliated with the Burdur Provincial Health Directorate must have the forms they filled out and signed (there is no need to apply to have them signed by the Burdur Provincial Health Directorate) signed only by the Head of Department and submit all four copies by the end of working hours on Friday, January 27, 2024 to their advisor in exchange for a signature. Applications of students who do not submit their documents by the specified date will not be considered.
