The forms to be used by students who want to do an optional internship are attached.

Note: Our students must fill out the Optional Internship Acceptance Form in 3 copies, the Optional Internship Agreement in 3 copies, and the Student Application Petition in 1 copy, and submit them to our Vocational School along with the occupational health and safety certificate and acceptance document at least ten business days before the start date of the internship.
